Skilled Nursing

The Skilled Nurse is a RN or LPN with training and experience in providing care outside the hospital or nursing home environment. The nurse is your health care team leader and coordinates all of the services provided in your home. The nurse also communicates frequently with your physician to update you plan of treatment.

Therapies

Physical Therapy, Occupational Therapy, Speech Therapy and Nutrition services are provided by a licensed therapist or licensed therapy assistant under the direction of the therapist and MD orders. Your therapist will design a patient specific plan of care to meet individual patient needs.

Medical Social Worker

The Medical Social Worker provides short-term counseling services, referral to and coordination with community resources and assistance with living arrangements, finances, and care-giver support.

Home Health Aide

The Home Health Aide provides intermittent personal care and assistance with ADL’s which include, but are not limited to, assistance with personal hygiene, light housekeeping, and small meal preparation. These services are delivered under the supervision of a registered nurse or physical therapist. Our aides have experience caring for patients in their home environment.

In emergency situations call 911 or the local emergency number.

Registered Nurses

A qualified nurse is on-call to accept patient calls, referrals for patient services as needed. This agency is not an emergency service.

After Hours Coverage
(24 x 7)

Coverage is available 24 hours a day, 7 days a week through an answering service and on-call procedure after normal office hours, weekends, and holidays.

Full-Time and Part-Time Field RN’s – all areas 

Responsible for direct care of a group of patients including coordination of care with other care providers.  Must be reliable and able to work independently. Applicant should possess strong clinical skills including IV therapy, wound care and clinical assessment. Home Health experience is preferred.  Kinnser knowledge is a plus.  Benefits package and SIGN ON BONUS for qualified applicants.
